Please note: All participants are required to read these rules and regulations before completing the registration form.
1.1 Each participant in the contest consents to the collection and use of the information, responses or opinions that they provide for the purpose of this Challenge. The participant consents to the Department of National Defence (DND) making the collected information available to the organizer for statistical and program evaluation purposes. The Health Promotion Director may also use this information as part of the Challenge's results published on their Websites and/or in other media. The Challenge's winners´ identities will be revealed on the March 1st, I Quit website, as well as other Canadian Forces (CF) media groups.
1.2 Any questions regarding the Official Challenge's Rules and Regulations should be directed to the Health Promotion staff located on CF bases and wings.
1.3 The information you provide will be retained in a Personal Information Bank which is currently under development for this program. Under the provisions of the Privacy Act, individuals have the right to the protection of, and access to, their personal information. Instructions for obtaining this information are outlined in the government publication entitled Info Source, a copy of which can be obtained at your local Service Canada Centre. Info Source is also available on the web at http://infosource.gc.ca/index-eng.asp.
All information will remain confidential and will be used only for statistical purposes. D FHP is responsible for all health promotion and disease prevention programs within DND, in partnership with the Director General Personnel and Family Support Services (DGPFSS). You are authorizing D FHP to publish your name, should you be a winner in the prize draw.
1.4 Because some information collected for the March 1st, I Quit! Challenge also relates to third party personal information, the participant hereby acknowledges and certifies that their supporters have consented to the collection and use of their personal information for the purpose of this Challenge.
2.1 All entries become the property of DND and will not be returned.
2.2 Each prize must be accepted as awarded and is not transferable or convertible to cash.
2.3 Maximum one contest prize per household.
2.4 In the event that two entries or more are submitted for the same participant, only the first entry will be considered and accepted.
2.5 By participating in this Challenge, an entrant agrees to the use of his/her name and address (city and province only) for publicity purposes relating exclusively to this Challenge without compensation.
2.6 By participating in this Challenge, participants agree to be bound by the Official Challenge's Rules and Regulations and the decisions of the organization's authorities.
2.7 The DND reserves the right to modify and/or cancel this Challenge in the event of technical corruption, virus, bugs, non-authorized human intervention or any other technical problems, which may arise from time to time beyond the control of DND, destroying the integrity of this Challenge. No liability or responsibility is assumed by DND resulting from user attempts to participate in this Challenge or if required, download any information necessary to participate in this Challenge.
2.8 Personal information collected through this Challenge (name, email address, etc.) will be used exclusively for the purposes of executing this Challenge. The information collected will not be transferred to any third party. All personal information submitted by participants who agree to participate in the evaluation will be kept confidentially and stored appropriately at the end of the Challenge for statistical and program evaluation purposes only. For participants who do not agree to participate in the evaluation of the campaign, all nominal information will be destroyed at the end of the Challenge.
3.1 The Challenge is open to residents of Canada, over the age of majority (in the province of residence) at the time of entry.
3.2 All participants must complete all of the required and/or mandatory fields on the relevant portion of the web entry registration form and/or brochure form. All information obtained for this Challenge will be used only for the purposes of conducting the Challenge.
3.3 To be eligible, entries must be received on or before February 29th, 2012 at 11:59 PM EST. Incomplete or late entries will not be accepted.
3.4 By entering, participants agree to abide by the Challenge's rules and the decisions of the organization's authorities, which shall be final. This Challenge is subject to all applicable Federal, Provincial, and Municipal laws and regulations.
3.5 If it is discovered in the ordinary course of running the campaign that one person has deliberately attempted to enter more than once, that person may be disqualified from the Challenge.
3.6 The participant in the Challenge must meet the following criteria to be eligible for the draw. Be a current user, or a former user of tobacco who quit using after September 1st, 2011.
3.7 Must be of the age of majority in the province in which he or she resides and either be:
3.8 The supporter must be of the age of majority in the province in which he or she resides and be a non-user of tobacco, or ex-user who has quit using before September 1st 2011.
3.9 Must be of the age of majority in the province in which he or she resides and either be:
3.10 When a person agrees to help a tobacco user in his or her bid to stop using, they thereby become an official supporter and automatically become eligible for the draw except under the conditions of ineligibility listed below.
3.11 The two (2) supporters of the winning participant will have to certify that, to their knowledge, their participant kept his/her commitment not to use tobacco during the month of March 2012.
3.12 Non-users of tobacco of more than 6 months (i.e. before September 1st, 2011) are not eligible to register as a "participant" in the Challenge. The only way they can participate is by acting as an official supporter to an eligible participant.
3.13 The following persons and their immediate family members cannot register to the Challenge, nor win any of the prizes: military and civilian employees of D FHP, CFMG PA, PSP managers, Health Promotion Directors, Health Promotion Managers, and Health Promotion Administrative Assistants and/or any other military or civilian employees involved with the organization of this Challenge.
4.1 Complete and sign the "Participant Registration Form" which can be found in the brochure, the local newspaper or on the website. An online registration form is available through the "March 1st, I Quit!" website (Internal). The participant can choose and click their HOME base from the dropdown list of all active CF bases or wings. (Military members are required to register under their HOME base even if deployed or on training at another base or wing.)
4.2 Find two (2) supporters. The supporters will need to certify that the participant remained tobacco-free during the month of March 2012.
4.3 Register online no later than February 29th, 2012 at 11:59 PM ET or deposit their completed registration form before February 29th, 2012 at 11:59 PM ET, in the box located at the Strengthening the Forces Health Promotion office. On some bases or wings, the ballot box could be located at the medical clinic, the CANEX store, SISIP FS or at any other designated location on the base or wing. (Health Promotion Directors will decide where the registration boxes will be located, and ensure they are visible and accessible to all participants. Registration boxes will be securely located as they are considered Protected A Information.)
4.4 Stop using tobacco before or by March 1st, 2012.
4.5 Remain tobacco-free at least until April 1st, 2012.
4.6 Former tobacco users who have quit using in the last 6 months must remain tobacco-free.
4.7 The winners will be tested (with a carbon monoxide monitor, if applicable) to prove that they have not been using tobacco during the month of March 2012. If the test can not be done for any reason, the winners will have to sign a statutory declaration. (Participants can use any tobacco cessation medication such as Zyban, nicotine patches or gum as the test is to detect carbon monoxide, which is an indicator of smoke exposure only.)
4.8 Winners will also have to answer a simple skill-testing question.
4.9 By signing the registration form or by submitting on-line registration, the participant confirms his/her status as being a tobacco user or former tobacco user (after September 1st, 2011). They declare that they have read and accepted the Challenge's rules. The participant also gives his/her authorization to the Director Force Health Protection (D FHP) to use his/her personal information in order to evaluate the March 1st, I Quit! Challenge. This information will remain confidential at all times. The participant also authorizes the D FHP to publish his/her name if they become a winner.
4.10 Only one registration form is allowed per candidate.
4.11 Registration is free and no purchase is necessary.
4.12 Candidates can withdraw at any time.
5.1 To enter, log on to the March 1st, I Quit! web site (internal site) during the campaign's promotion, complete and submit the on-line entry form or fill in the registration form brochure. Complete all mandatory fields or lines of the registration form. A fully completed on-line entry form will constitute one (1) Internet entry to the Challenge. Maximum one (1) entry per person/participant (either the brochure or on-line registration) will be accepted.
5.2 This Challenge starts January 18th, 2012 at 12:01 AM ET and ends on February 29th, 2012 at 11:59 PM ET.
5.3 No responsibility is assumed by DND, for any disruption to telephone network or lines, computer on-line systems, servers or providers, computer equipment, software or failure of any entry to be received because of technical problems or traffic congestion on the Internet or at the website, or any combination thereof, as well as lost, misdirected, illegible, or late entries which are not received for any reason.
6.1 There will be one (1) national draw and twenty-eight (28) local draws. All draws will be held on April 1st, 2012.
6.2 The national draw will be held at CF H Svs Gr, HQ located at 1745 Alta Vista Dr. in Ottawa.
6.3 The first national draw will determine the winners among participants who have quit using tobacco on March 1st, 2012, and former tobacco users who have quit using after September 1st, 2011.
6.4 The second national draw will determine the winners among all supporters.
6.5 There are four (4) national prizes; three (3) for participants and one (1) for supporters; and eighty-four (84) local prizes; fifty-six (56) for candidates and twenty-eight (28) for supporters.
6.6 The selected winning participants will be notified by phone and/or email.
6.7 To win, the winners may be required to show proof of age of majority and will be required to sign a declaration and release form, releasing the Department of National Defence from all liability. The winners will also have to pass a carbon monoxide test or sign a declaration that they have not used tobacco during the month of March 2012. They will also be given a simple skill-testing question.
6.8 In the event that a selected winning participant or winning supporter cannot be contacted by phone and/or email, after two (2) attempts within fourteen (14) days of prize notification, the Health Promotion Director reserves the right to select another winner.
7.1 The Surgeon General, CANEX and SISIP FS Presidents along with the Health Services Group HQ and/or their official representatives will draw from the registration lists amongst all participants´ names received. DHPHP will inform the Health Promotion teams of potential winners; HP staff on bases and wings will be responsible to verify the eligibility of potential winners.
7.2 National prizes for ex-tobacco user participants are as follows:

8.1 The local draws will be held on April 1st, 2012, at each of the twenty-eight (28) locations across the CF bases and wings located throughout Canada and one in Europe. There will be two (2) local prizes for the participants who have quit using tobacco and one (1) prize for the supporters that will be drawn at each twenty-eight (28) locations.
8.2 CF base and wing leadership representatives, health care staff representatives, CANEX and SISIP FS Managers, PSP Managers and local partners will be invited by HPD to participate in the drawing ceremony.
8.3 Two (2) local draws are planned. The first local draw will determine the winner among participants who have quit using tobacco on March 1st, 2012 or former users who have quit using tobacco after September 1st, 2011. The second local draw will determine the winner among the supporters.

9.1 The Surgeon General retains final authority as to decisions related to candidate registration and the assessment of their status as a user or non user of tobacco.
