CASNR 09.001 - February 11, 2009
OTTAWA – The Board of Inquiry (BOI) has been completed into the crash of Cormorant helicopter CH149914 during a night boat exercise offshore of Canso, Nova Scotia on July 13, 2006.
Killed in this tragic accident were Sergeant Duane Brazil, Master Corporal Kirk Noel and Corporal Trevor McDavid.
A Board of Inquiry is an administrative investigation whose primary purpose is to investigate a matter of concern to the chain of command and to make recommendations for preventive or corrective actions that will help prevent recurrence in the future. This process is separate and independent from the Flight Safety investigation already conducted for this accident. The BOI report cannot be publicly released in its entirety due to ongoing legal proceedings associated with this accident and because some portions contain personal information.
As a result of this accident, the Air Force has implemented over 60 preventive measures to improve the safety of the Search and Rescue personnel who perform this vital and demanding mission.
